
The Wellbeing Team works together for the benefit of patients (18+) helping them to identify what matters to them and supporting them to improve their health and wellbeing.
Social Prescribing connects people to activities, groups, and services in their community to meet the practical, social and emotional needs affecting their health and wellbeing.
Social Prescribing offers short term interventions, connecting people (18+) to support with:
- Social groups, Hobbies, and Interests
- Employment and volunteering
- Benefits, Housing, and financial problems
- Social Isolation and loneliness
- Low mood, stress, anxiety, improved wellbeing– those willing to consider alternate options to clinical interventions/medication, eg. support groups, peer support, social activity, self-help, and physical activity.
EXCLUSIONS
- Social Prescribing is not a crisis service - all tasks/referrals will be triaged with priorities assessed.
- We will not accept referrals for patients who are in crisis/suicidal that need clinical mental health support.
- Referrals will be returned to referrer if no consent given, or if not appropriate for Social Prescribing, for example:
- too complex for Social Prescribing to be beneficial or not ready to engage.
- require mental health support for severe and enduring mental health problems.
- already receiving support from Whitegate Drive Health & Wellbeing service.

Referrals will be returned to referrer if no consent given, or if not appropriate for Social Prescribing, for example: too complex for Social Prescribers to be beneficial or not ready to engage, require mental health support for severe and enduring mental health problems, already receiving support from Whitegate Drive Health & Wellbeing service. Referrals can only be accepted for patients registered at the following GP surgeries: St Paul’s Medical Centre, Adelaide Street Family Practice and South King street Medical Centre. All other referrals will be declined.
