Care Coordination - Blackpool Central West PCN

The Wellbeing Team works together for the benefit of patients (18+) helping them to identify what matters to them and supporting them to improve their health and wellbeing.

Our role as Care Coordinators is to help improve patients’ health and wellbeing. Care Coordinators connect people back into health and social care when they have complex needs, lack of support and multiple appointments.

  • We do this by supporting patients, their families, and carers to manage their own health and social care by finding out what matters most to them.
  • We may need to work closely with other health and social care professionals and may navigate these patients to appropriate services.
  • We speak to patients, their families, and other professionals about their current circumstances and how they could be improved.
  • Following this conversation, we may be able to work with the Carers Centre, Community Hub, Trinity Hospice, FCMS, Adult Social Care, Care Home Support Team, and the Frailty Service to create a more coordinated approach for the most vulnerable patients.

Service access criteria:

18 years old and up. Referrals can only be accepted for patients registered at the following GP surgeries: St Paul’s Medical Centre, Adelaide Street Family Practice and South King street Medical Centre. All other referrals will be declined.
