The Wellbeing Team works together for the benefit of patients (18+) helping them to identify what matters to them and supporting them to improve their health and wellbeing.
Health and Wellbeing Coaches support people to make lifestyle change that in turn improve how they manage their physical and mental health. They work with people to develop the knowledge, skills, and confidence to change their health-related behaviours.
Support around self-management of health conditions and help overcome barriers to improving health, using coaching techniques to improve motivation and provide education to enable patients to access further health and wellbeing services.
Health and Wellbeing Coaching is a non-clinical service designed to support individuals in making positive lifestyle changes. To benefit from this service, patients must be committed and willing to change their behaviours. Participants are expected to commit to six fortnightly coaching sessions. EXCLUSION CRITERIA: Eating disorders, uncontrolled physical health issues including those still under investigation, uncontrolled mental health, those requiring clinical input. Please note that Health and Wellbeing Coaches are not registered dietitians and cannot provide advice for those with specific dietary needs. For patients seeking support to lose weight, the referring clinician should offer Tier 2/3 National Weight Management Programmes if eligible and where appropriate. Health and Wellbeing Coaches cannot provide advice on weight loss injections or weight loss surgery. Patients must be 18 years or older. Referrals can only be accepted for patients registered at the following GP surgeries: St Paul’s Medical Centre, Adelaide Street Family Practice, South King Street Medical Centre All other referrals will be declined.
