Health and wellbeing coaches support people to increase their ability to self-manage, motivation levels and commitment to change their lifestyle. They work with people to set personalised goals and change their behaviours.
They work with people with physical and/or mental health conditions and those at risk of developing them.
Health and wellbeing coaches can be an effective intervention for people experiencing a range of long-term conditions, including respiratory, cardiovascular (including type 2 diabetes and hypertension), and stress/low mood, hyperlipidaemia and non-diabetic hyperglycaemia. They can also support people with weight management, diet and increasing activity levels.
Health and Wellbeing Coaching is all about goal settings, if you have health goals you are determined to reach and would like some support along the way. Coaching service varies depending on the specialism of the team at your GP service. You will have an initial screening appointment before the programme of a few weeks of coaching, if eligible.
Criteria
- 18+ and registered with local GP.
- Don't offer specific support for loneliness, homelessness, talking therapy/counselling & completing documentation.
- Service user must show commitment to working towards health goals.
