The Hillingdon Community Team for People with Learning Disabilities (CTPLD) offers specialist support for adults with learning disabilities living in the London Borough of Hillingdon. Our team works closely with Hillingdon Council and provides care across both primary and secondary health services, supporting individuals, their families, and carers.
Our multidisciplinary team includes:
- Psychiatry
- Nursing
- Psychology
- Speech and Language Therapy
We offer a range of interventions, including:
- Risk assessments and risk management plans
- Person-centred care plans
- Mental health and psychological assessments and interventions
- Assessment and interventions for people with behaviours that challenge
- Screening and early intervention to prevent unplanned health or other services (including hospital admissions)
- Signposting and referrals to other appropriate services/teams, including the third sector
- Training for other service providers, such as GPs, on understanding the health needs of people with learning disabilities
Our goal is to provide holistic, person-centred care that helps adults with learning disabilities live healthier, more independent lives in their community.
Service opening times: Monday to Friday, 9am to 5pm (closed on public holidays). Clinicians may work outside these hours according to need.
