Our Social Prescribing Link Worker team work alongside health professionals based in GP practices. The team work closely with GPs, Nurses, Pharmacists to assist patients with any 'non-medical' needs that may be having an impact on your physical/mental health.
They offer a 1-1 personalised service via telephone or face to face, taking the time to listen and understand 'what matters to you'. It may be that you are facing issues with finances, housing or employment, suffering domestic violence or using drugs/alcohol. You may have a limited support network and are socially isolated. You may live with a long term condition, have caring responsibilities or are bereaved. You may like to look at volunteering opportunities. Your Social Prescribing Link Worker will work with you to create a care plan, which may include signposting/referrals into local services and organisations within the community.

We only accept referrals for patients who are 18+ without complex mental health needs. We are not an emergency or crisis service.
