Barking & Dagenham, Havering and Redbridge Falls Clinic is a multidisciplinary team that works across the boroughs of Barking and Dagenham, Havering and Redbridge to help people aged 50 and over (unless medically proven at risk of falls) who are at risk of falling. The service assesses and treats people with falls risk factors such as a history of slips/trips, balance problems, taking multiple medications, or difficulty rising from a chair. Our aim is to prevent falls, reduce slips and falls, lower hospital and A&E admissions, and minimise fear of falling so people can live more independently and safely. The Falls Clinic team includes a Falls Consultant, Physiotherapist, Occupational Therapist, Nurse, Falls Practitioner and Integrated Care Assistants. We provide medical review and optimization of medications, strength and balance assessment and training (with options for in-person and virtual classes), home environment assessment and modification advice, equipment recommendations, and tailored coping strategies for patients and carers. The service offers in-clinic sessions as well as home visits when appropriate. Referrals can come from self-referral, GP, consultants, community matrons, A&E staff, community therapy teams, social services, voluntary organisations and other professionals. How to refer: GP referrals via EMIS/Elemental for routine referrals; non-urgent referrals via the SPA team at nelft.haveringreferrals@nelft.nhs.uk; urgent referrals follow existing pathways. For enquiries, contact Tel: 0300 300 1851 (Option 1 for Falls Service, Option 2 for Falls Practitioner Service) or email BHRFallsservice@nelft.nhs.uk.
