The Integrated Care Team (ICM) in Havering provides coordinated, multi-disciplinary support for adults over 18 with complex health and social care needs. Our goal is to ensure you receive the right care, in the right place, at the right time.
We work closely with GPs and other health and social care providers to deliver a high-quality package of care for vulnerable adults. The service is divided into three localities across Havering, operating from local clinics and health centres.
- Who is it for? Adults (18+) in Havering with complex health and social care needs.
- How does it work? Our team coordinates care with your GP and other professionals, offering support tailored to your needs.
- Where is it delivered? Local clinics and health centres in Romford, Harold Hill, and Hornchurch.
- Access: Referrals are accepted from GPs, health and social care professionals, and self-referrals from patients and clients.
- Opening times: Day service: Monday to Sunday, 8.30am - 5pm. Night service: Monday to Sunday, 8.30pm - 5am.
