Integrated care team (Havering)

Integrated care team (Havering) helps adults aged 18 and over with complex health and social care needs receive the right care, in the right place, and at the right time. The team coordinates a multi-disciplinary, high-quality package of care in close collaboration with GPs and other health and social care providers to support vulnerable adults to stay independent and well in their own homes. The Integrated Care Team is divided into three localities across Havering, operating from local clinics and health centres. What you can expect: • Referrals: Accepted from GPs and health and social care professionals. Self-referrals are also welcome. • Access points: Care is delivered from three local sites (clinic-style settings) across Havering. • Opening times: Day service 8:30am–5:00pm; Night service 8:30am–5:00pm.
