Havering Liberty PCN's Social Prescribing service connects adults aged 18 and over to local non-medical support for issues impacting their health and wellbeing. This includes help with loneliness, bereavement, housing, financial advice, carer support, substance misuse, digital inclusion, domestic abuse, gambling, ageing well, food access, low-level mental health concerns (anxiety, panic attacks, low mood, stress), smoking cessation, weight management, employment, training, and volunteering. Social Prescribers work in GP practices to create personalised support plans and can physically introduce individuals to community groups, activities, and statutory services. The service is for residents living in Havering and registered with one of the following GP practices: Hornchurch Healthcare, Maylands Healthcare, Cranham Village Surgery, Haiderian Medical Centre, or Avon Road Surgery. Access the service via a self-referral form or by asking your GP for a referral.
