Need help getting your North Kent adult community care enquiry to the right team?
Care Coordination Centre is for adults using North Kent Adult Community Services in Dartford, Gravesham, Swanley and Swale. It provides one access point for enquiries and referrals, excluding community wards, and directs them to the appropriate clinical team.
