This dedicated service provides support and case management for people with heart conditions, including cardiac rehabilitation, management of heart failure (left ventricular systolic dysfunction), and cardiac diagnostics. Care is delivered in patients' own homes and other community settings. The service covers Dartford, Gravesham, Swanley, and Swale. Available Monday to Friday, 8am to 5pm. Access is via their Care Coordination Centre.
