North Kent Community Diabetes Service provides specialist advice and support to help you manage diabetes effectively. The team supports people with Type 1 and Type 2 diabetes, offering education, practical guidance, and clinical management pathways. You can choose to attend clinics or have the team visit you at home; home visits are available for housebound patients. The service works closely with your GP, practice nurse, and consultant to ensure coordinated care and to help you live healthily with diabetes. For Type 2 diabetes, X-PERT education is provided as six 2½‑hour sessions over six consecutive weeks at Sheppey Community Hospital or Sittingbourne Memorial Hospital, with morning, afternoon or evening options and six‑month and one‑year follow‑ups. For Type 1 diabetes, Type 1 Diabetes Education (T1DE) involves four full days over four consecutive weeks to teach insulin dose adjustments in line with carbohydrate intake and activity. The service is available Monday to Friday, 8am–5pm. If you’re in Swale, diabetes education is available as required. Access is via the North Kent Care Coordination Centre; a Referral Form is available and once completed, the form should be emailed to vcl.dgss-ccc@nhs.net.
