Our navigators will guide you, and your family and carers, through accessing essential services and local support, enabling you to find the right opportunities and make your own choices.

If you are a patient at Darent Valley Hospital, our hospital navigators can provide you with practical support to ensure that your home is safe, and care is in place so that you can be discharged when you are ready.

We can help with:

•  Supporting you in the best way - providing access to the service through home visits, face-to-face appointments, telephone or email.
•   Assessing your needs - listening to your needs and identifying what kind of help and support would benefit you.
•   Access to financial support and advice - helping you manage money and claim any income and benefits you may be entitled to.
•   Maintaining and adapting your home - arranging mobility adaptations, equipment aids and assistive technology to support you to live safely and independently in your home.
•   Applying for other services, grants and support - helping you to complete a wide range of assessment and application forms.
•   Building your social networks - exploring peer support groups, befriending and creative activities, to make social connections and develop your interests.
•   Accessing other support - helping you find support for a range of needs including domiciliary care, housing, transport, gardening, handyman services, shopping etc.
