WorkWell is for people aged 16 or over in Leicester, Leicestershire and Rutland who have a disability or health condition and the right to work in the UK. A work and health coach offers light-touch, personalised support and can connect you with local NHS, community and employment support.
- Initial assessment of health-related barriers to work
- Return-to-work or thrive-in-work plan covering physical, psychological and social needs
- Advice on workplace adjustments
- Employer liaison if you consent to share your plan
- Signposting or referrals to local support, such as health, care, training or debt advice

The patient must be over 16 years of age and living in or registered with a GP or Job Centre in Leicester, Leicesterhsire and Rutland. They must have the right to work in the UK.
