Workwell - Herefordshire

There is strong evidence linking health and wellbeing outcomes to employment. WorkWell is an early intervention and prevention programme that aims to help people to improve their term health and wellbeing outcomes and reduce pressure on local health services.

The WorkWell programme is designed to support individuals with health conditions in staying employed or returning to work by offering holistic and personalised support. The programme's primary aims are to promote employment retention, facilitate return to work, improve both mental and physical well-being, reduce health-related employment barriers, and foster long-term independence for participants. 

Participants will receive personalised and holistic support through return to / thrive in work plans and referrals / signposting into local groups and services.

Service access criteria:
  1. Eligible to work in the UK, and
  2. Reside within the ICB area, or
  3. Be registered with a GP within the ICB area.
  4. 16 or older
  5. Currently on sickness absence and at risk of leaving employment / or has just left employment due to long term sickness
  6. Has motivation / confidence to engage in the WorkWell programme

 

Please note referrals may be rejected if any of the following are true:

  1. Under 16
  2. No motivation or confidence to engage
  3. Complex mental health such as self-harming, suicidal ideation and / or intent, hearing voices
  4. People with urgent needs
