Move Together Buckinghamshire

Move Together Buckinghamshire is a free programme designed to help residents aged 50+ living with a long-term health condition to become more active and improve their physical and mental wellbeing.

This programme offers up to 12 months of personalised support to help you move more in a way that suits you, with professional guidance every step of the way. It includes a 

  • personalised physical activity plan tailored to your needs and abilities,
  • help accessing local, affordable activity sessions, 
  • and ongoing encouragement to build confidence and stay motivated.

Whether you’re just getting started or looking to return to exercise, we’ll help you find the right pace and activities for you, no matter your starting point.

Service access criteria:
  • Over the age of 50 years old
  • Live in Buckinghamshire or be registered with a Buckinghamshire GP practice
  • Currently inactive or fairly inactive (do less than 30 minutes of activity a day)
  • Motivated to increase activity levels to improve their health and wellbeing.
  • Living with a long term health condition (see the list of approved conditions below).

List of eligible long term health conditions:

- Alzheimer’s
- Arthritis and Musculoskeletal Conditions
- Asthma, COPD  (and wider lung conditions)
- Cancer
- Dementia
- Diabetes (controlled)
- Hypertension
- Limited Mobility Conditions
- Chronic Fatigue Syndrome
- Mental Health Conditions
- Multiple Sclerosis
- Osteoporosis
- Parkinson's
- Stroke (stroke must have occurred more than 3 months ago)
- Falls (for people at risk of developing frailty)
- Heart Conditions
- Chronic Kidney Disease 
- HIV
- Fragility fracture

Exclusion Criteria

  • Unplanned or unexpected admission to hospital because of a health condition in the last 4 weeks. If this is the case, seek advice from a healthcare professional before joining.
  • Acute coronary syndrome (a heart attack within the last 6 weeks or unstable angina)
  • Severe or poorly controlled asthma
  • Severe COPD
  • Currently accessing cardiac or cancer rehab or pulmonary services
  • Currently undergoing wider tests regarding the long term health condition(s)
  • Blood Pressure >180mmHg (systolic) or >100mmHg (diastolic)
  • Resting heart rate >100 bpm
