Sevenfields PCN's Social Prescribing service is piloting an alternative method of social prescribing with group consultations. Our vision is that these group sessions will offer opportunities social connection, peer support and provide opportunity for group discussion and consultation with an SPLW in a group setting.
Criteria of suitable participants for pilot project:
- Individuals who would like to learn more about how then can support their health and well-being in their communities.
- Individuals who are feeling lonely or isolated and may benefit from peer support.
- Individuals who are happy to contribute within a group setting, listen to others and share their own experiences of well-being.
To access this programme, a referral for Social Prescribing should be made via a Sevenfields PCN GP practice.
