Social prescribing is a service available through Ravensbourne GP practices that helps patients address non-medical factors affecting their health and wellbeing.
Social Prescribing Link Workers work alongside patients to identify issues such as social isolation, financial difficulties, housing concerns, low-level mental health challenges, or lifestyle changes. They provide personalised support and connect individuals with appropriate community groups, services, and activities to help improve their overall quality of life.
This service is free and available to patients registered with Ravensbourne practices. Speak to a member of your GP practice team to find out more or request a referral.

Inclusion Criteria:
- People over 18 years old, or 17 and transitioning to adult services
- People with one or more long-term conditions
- People who need ‘low-level’ support with their mental health
- People who are lonely or isolated
- People who have complex social needs which affect their wellbeing.
- Patients registered with Ravensbourne PCN practices (Rushey Green Group Practice, Baring Road Medical Centre, ICO Health Group - Moorside Clinic)
Exclusion Criteria:
- People who do not fall into one of the accepted groups
- People who are at risk of harming themselves or others
- People with severe and enduring mental health conditions or substance use
- People in palliative care
- People who are not willing to engage with the Social Prescribing Link Worker
- People who are being referred for mainly clinical reasons
