Active Wellbeing is a programme for adults with a diagnosed severe and enduring mental health illness (SMI). It is funded by NHS Wandsworth CCG and delivered in partnership with Mind in Brent, Wandsworth and Westminster.
If referred by a care coordinator, support worker or GP (or via self-referral), you can receive 10 weeks of one-to-one exercise support with an exercise instructor. Activities are tailored to you and may include walking, jogging, park workouts, football, or using the gym.
To join, speak to your care coordinator, support worker or GP who can refer you using the referral form (if you are eligible). You can also self-refer by contacting the team by phone or email.
