**Stroke Recovery Service Overview**
Our Stroke Recovery Service is designed to support stroke survivors, including those who have experienced TIAs (mini-strokes), as well as their families and carers. We focus on identifying your unique support needs and priorities to assist you on your recovery journey.

- **Home Visits & Phone Calls**: A coordinator will arrange to connect with you through regular phone calls or in-person visits to assess your daily activities and determine any additional support you may need.

- **Community Access**: Assistance in navigating community-based support services.
- **Support for Carers**: Dedicated resources for family members and caregivers.
- **Signposting**: Guidance to other relevant organizations to further aid your recovery.
