Age UK CAP - Hospital Discharge and Admission Avoidance service

At Age UK Cambridgeshire and Peterborough (Age UK CAP), we're here to help. We believe No One Should Have No One. We offer practical support and help to make social connections to reduce feelings of loneliness or isolation.

Hospital Discharge and Admission Avoidance service operates 7 days a week, 365 days a year 10am-4pm. This service can react quickly to a discharge home and support the individual with the following:

  • Furniture and bed moves
  • Key safe fitting prior/upon discharge
  • Grab rail fitting
  • Checking home is safe and warm
  • Support to settle home on day of discharge
  • Essential/Urgent food shops
  • Essential medical needs/prescription collections
  • Making GP or hospital follow-up appointments
  • Helping to support reinstate suspended services
  • Urgent welfare visits and/or welfare telephone calls
  • Support to identify and navigate to longer term services needed (health, social care, Age UK CAP services or other voluntary/community organisations)
  • Person-centred and a complete wrap-around service
  • An information pack

For those that may need support to prevent an admission/re-admission to support the older person to remain living at home safely and independently, we can react to urgent calls or failed calls with an urgent welfare home visit.  

If longer-term practical support is needed, we refer to our other services, or externally as required and discussed with the individual. For example, our Handyperson service can visit to do a Safety around the home assessment and look at mobility around the home offering further support and advice. Our Sharing Time service might be appropriate for ongoing conversation and friendship support to reduce isolation and loneliness, improving the individual's overall wellbeing.
