Tiverton Wellbeing offers personalised one-to-one support from Social Prescribers to help patients improve their health and wellbeing. The team works closely with GPs, other healthcare professionals, and the local voluntary and community sector to signpost people to suitable services and activities in the area.
When you meet with a Social Prescriber, they will listen, explore what matters most to you, and discuss practical options to help you feel more in control of your health and wellbeing. They may introduce you to local services and organisations for additional support and help you identify sustainable, small steps you can take to feel better.
What we can signpost you to:
- Employment, education, training and volunteering opportunities
- Housing, benefits and financial support advice
- Healthy living and weight loss support
- Social, hobbies and interest groups
- Therapy and support groups
What to expect:
- One-to-one support in a confidential, non-judgemental setting
- A plan tailored to your needs with practical steps and goals
- Help navigating local services and overcoming barriers to access
HOPE Programme: A six-week programme (Help Overcoming Problems Effectively) focusing on mindfulness, stress management, goal setting, fatigue management, and other strategies to improve wellbeing. The programme runs based on demand and waiting lists may apply.
How to access:
Self-refer via the Joy referral process (click the Joy referral button on the page, then select "Make a referral"); you must be a registered patient at Amicus Health (Clare House Surgery, Bampton Surgery and Mid Devon Medical Practice at Witheridge, Morchard Bishop and Cheriton Fitzpaine) or Castle Place Practice to be accepted.
- Referrals can also be made by a GP or another organisation.
