Everyone Active GP Exercise Referral Scheme
The GP Exercise Referral scheme is aimed at adults (16 years and over) with a low or moderate risk long-term health condition. The scheme has been designed to help individuals self-manage their condition more effectively and with confidence through physical activity.

What does the GP Exercise Referral Scheme involve?
Through an initial consultation our GP Exercise Referral instructor will find out what is important to you and help you find activities you will enjoy! A 6 and 12 week review will enable the instructor to support you through your first 12 weeks to a healthier lifestyle.
Activities vary depending on your interests and can include:
· Forever Fit – a low impact exercise class
· Group exercise classes
· Swimming or water-based activities such as Aqua Aerobics
· Gym based exercise
· Condition specific activities such as Good Boost technology or Escape Pain for MSK conditions.
· Access to Everyone on Demand – Video workouts that can be used in the gym or at home
Please note all of these sessions can be accessed as a pay as you go customer without joining the scheme, however you will not have the appointments/support from the GP Referral Instructor.
You will pay a reduced membership fee of £22.49 per month for a 6 month period. The price then increases to either our standard monthly membership (£32.49 per month) or, if eligible, our concession membership (£26.99 per month). Please note these prices can be subject to change. You can cancel as per the membership terms and conditions.
What is the referral process?
A GP, healthcare professional or Social Prescriber will refer you via an online referral form. Once submitted, you will receive an email with a link to complete your GP Referral membership profile and Health Screening Questionnaire. Once these are completed the GP Exercise Referral Instructor receives the information and will contact you to arrange a suitable day/time for your consultation. After the initial consultation you will also be contacted to book a review at week 6 and 12, as well as complete questionnaires at these stages.
Eligibility
You must be 16 years and over and assessed by the referrer as inactive in relation to ACSM’s guidelines. In this case, inactive is classified as doing less than 30 minutes of moderate activity (such as a brisk walk) two days a week and exhibit at least one of the criteria listed below in either the low or medium risk groups.
