Thr1ve Social Prescribing for Young People offers short-term, person-centred support to help young people connect with their communities and improve mental health and wellbeing. The service is delivered by the Thr1ve team within Chesterfield FC Community Trust and provides both 1:1 support and small group sessions for young people aged 14–18, and young adults aged 19–25 (no diagnosis needed; additional support needs accepted). Our aim is to connect young people with suitable services, activities, education or training, and volunteering opportunities, helping them gain confidence, develop independence skills and engage with new activities and communities. How it works is tailored to the individual: an initial meeting to identify priorities, followed by a simple, personalised plan. Referrals can be made by the young person, a parent/guardian, school nurses, GPs or other professionals, and a referral can be made via the Joy platform. The service covers areas including Chesterfield, Glossop, Buxton, North-East Derbyshire, Bolsover and Amber Valley, with a full list of participating surgeries available on the referral page.
