The wellbeing team consists of 3 Health and Wellbeing Coaches. The team provides personalised, patient focused and holistic support delivered over 6 appointments, giving patients the time and confidence to work on the underlying issues which are affecting their overall health and wellbeing.
Please note that for patients to be referred to us, they must contact their GP surgery to request a referral. No GP appointment is required, they can request a referral from any member of staff.
Patient referrals are triaged internally to the most appropriate member of staff and work with patients to explore individual’s needs by developing a plan to help create positive life changes through signposting, goal setting, motivation and empowerment. We have a comprehensive service directory to signpost patients to the appropriate services, groups and organisations that would benefit them for practical and emotional support.
The aim is to increase knowledge, skills and confidence in order to achieve their self-identified health and wellbeing goals and to help patients to have more control over their own health and find ways to improve how they feel in a way that suits them.
