Live Well Leeds offers community based support to individuals aged 18+ experiencing mild to moderate mental health issues. We are free and work in partnership with 14 delivery network partners to provide support across the city.
We provide a package of support in the community for up to 6 months including groups, welcome cafes (drop in), 1-1 support and volunteering opportunities all of which are focused on recovery, maintaining wellbeing and social inclusion. 1-1 support is goal focused and person centred, encouraging service users to create goals to improve and/or maintain their mental health and work towards them.
Once a referral is made, service users will be allocated a mental health support coordinator who will complete an assessment to better understand their support needs, identify goals and areas of change. Service users can then access any of the Live Well Leeds provision.

Adults aged 18+ (17+ if they are transitioning from children’s mental health services).
Mild to moderate support needs to manage or recover from their mental health.
The primary presenting supporting need is for their mental health.
Live in Leeds Metropolitan District or registered with a Leeds GP as defined by NHS Leeds Clinical Commissioning Group (CCG).
Live Well Leeds cannot take referrals for individuals with a primary diagnosis of Dementia or Learning Disability and these individuals will be signposted to appropriate support services.
