
Youth Social Prescribing Service (Amber Valley CVS)
What is social prescribing?
Social Prescribers work closely with GPs to assist people with their non-medical needs. We do this by helping individuals to identify 'what matters to them', and working collaboratively to build a care plan. This may include introducing them to community groups or activities, supporting them to meet new people, and accessing advice and information about a particular issue.
Our hope is for individuals to have a better quality of life; ultimately supporting them to improve their overall health and wellbeing by connecting them to community groups and statutory services. For example; social groups like youth clubs, art and craft groups, sports programmes and much more.
This service is provided in partnership with the Alfreton, Ripley, Crich and Heanor (ARCH) Primary Care Network, which includes the following nine GP practices:

- Ivy Grove Surgery
- The Park Surgery
- Kelvingrove Medical Centre
- Brooklyn Medical Practice
Who can be referred to this service?
Our social prescribing service supports young people aged 11-18 who are registered with a GP practice in ARCH Primary Care Network, and are experiencing low level anxiety, low mood, confidence, self image or social difficulties that are impacting on their life.
Referrals can be completed by:
- GP practices in ARCH Primary Care Network
- Swanwick Hall School
- The Ripley Academy
- David Nieper Academy
- Frederick Gent School
- Heanor Gate Spencer Academy
- Aldercar High School
- Derbyshire Public Health Nurses
- Elective Home Education Team (Derbyshire County Council)
- Child and Adolescent Mental Health Services (CAMHS)
Aged 11-18 and experiencing low level anxiety, low mood, confidence, self image or social difficulties that are impacting on their life. The young person must not have severe or complex mental health needs. If the young person is on the waiting list or receiving support from CAMHS, please discuss with us before making a referral to social prescribing. Referrals will only be taken from members of staff at ARCH Primary Care Network GP practices, Swanwick Hall School, The Ripley Academy, David Nieper Academy, Frederick Gent School, Heanor Gate Spencer Academy, Aldercar High School, Derbyshire Public Health Nurses, the Elective Home Education Team, and CAMHS.
