Our Health and Wellbeing team at Barnsley Healthcare Federation is here to help you live a healthier, happier life.
We offer two key services:
- Health and Wellbeing Coaches: Work with you to set and achieve personal health goals, whether you want to improve an existing condition, prevent new ones, eat healthier, get more active, or manage a long-term illness. Together, you'll create a personalised plan with manageable steps, guided by what matters most to you.
- Social Prescribers (Link Workers): Support you with social challenges that affect your wellbeing, such as loneliness, money worries, language barriers, housing, family issues, and more. Our Link Workers listen, take a holistic approach, and connect you to local community groups, voluntary organisations, and statutory services for practical and emotional support. They can even help you start new groups or make existing ones more accessible.
How to access:
- You can be referred by your GP, reception team, or other local agencies (pharmacies, hospital teams, social care, housing associations, etc).
- You can also request to see a coach or social prescriber directly by contacting your surgery, emailing us, or downloading a self-referral form from our website.
We offer home visits and work closely with you to find local activities, benefits, and services that make life easier and better.
Our team is based in doctors’ practices across Barnsley, and we’re committed to supporting you in every aspect of your health and wellbeing.
