Move Together Buckinghamshire
Supporting you to move more, feel better, and stay independent
Move Together Buckinghamshire is a free programme designed to help residents aged 50 and over, who are living with a long-term health condition, to be more active and improve their physical and mental wellbeing.
Funded by Buckinghamshire Council’s Public Health Team and delivered by Active in the Community (AITC), this three-year initiative is all about helping people move more in a way that suits them — with professional support every step of the way.
What’s included?
If you're eligible, you'll receive up to 12 months of support, including:
- A personalised 12-week physical activity plan, tailored to your needs and abilities
- One-to-one guidance from a trained adviser
- Help accessing local, affordable activity sessions
- Ongoing encouragement to build confidence and stay motivated
Whether you're just getting started or looking to return to exercise after a break, we'll help you find the right pace and activities for you.
Who can join?
The programme is available to residents who either:
- Live in one of the 10 Opportunity Bucks wards in Buckinghamshire (postcode checker here), or
- Are registered with one of the following 17 Deep End GP practices:

- Cressex Health Care
- Wye Valley Surgery
- Priory Surgery
- Carrington House Surgery
- Cherrymead Surgery
- Gladstone Surgery
- Riverside Surgery
- Berrycroft Community Health Centre
- Tower House Surgery

- Chiltern House Medical Centre
- Desborough Surgery

- Over the age of 50 years old
- Currently inactive or fairly inactive
- Motivated to increase activity levels to improve their health and wellbeing.
- Resident of one of the 17 registered GP practices (stated above)
- Living with a long term health condition (list of eligible conditions below)
- Alzheimer’s
- Arthritis and Musculoskeletal Conditions
- Asthma, COPD (and wider lung conditions)
- Cancer
- Dementia
- Diabetes (controlled)
- Hypertension
- Limited Mobility Conditions
- Chronic Fatigue Syndrome
- Mental Health Conditions
- Multiple Sclerosis
- Osteoporosis
- Parkinson's
- Stroke (stroke must have occurred more than 3 months ago)
- Falls (for people at risk of developing frailty)
- Heart Conditions
Exclusion Criteria
- Severe or poorly controlled asthma
- Severe COPD
- Currently accessing cardiac or cancer rehab or pulmonary services
- Currently undergoing wider tests regarding the long term health condition(s)
- Blood Pressure >180mmHg (systolic) or >100mmHg (diastolic)
- Resting heart rate >100 bpm
