Derbyshire Health and Wellbeing team

The Derbyshire Health and Wellbeing team offers coaching and personalised support to help people aged 16 and over in Derbyshire (excluding Derby City) to improve their health and wellbeing. The team works with a wide range of people, including those who have disabilities, experienced poor health (including mental ill health) or long-term health conditions, and aims to help you create a clearer picture of what a good life looks like for you. They begin by getting to know you, listening to what matters most to you, and identifying your goals, skills and passions. They then work with you to overcome barriers, build confidence, and pursue practical steps to reach your wellbeing goals. The team emphasises a person-centred approach: they will not try to fit you into a fixed service, but will tailor support to your needs and preferences, focusing on independence and resilience. Part of their role is to help you connect with your local communities and social networks, meet new people or reconnect with others, and access information and advice. They also provide signposting to more appropriate services or professionals when needed, and can arrange referrals or introductions to other agencies as required. Access is via an initial contact form (to sign up for an account and manage requests) or by contacting the team directly by email or phone. If you prefer, professionals can use the form to introduce a client or patient to the health and wellbeing team. The team can respond with information and advice, refer to other services, offer a face-to-face or telephone discussion, or explain why an introduction may be declined.
