Torquay PCN Wellbeing Team

The wellbeing team consists of Social Prescribers, Health and Wellbeing Coaches and Occupational Therapists specialising in Mental Health.  The team provides personalised, patient focused and holistic support delivered over 6 appointments, giving patients the time and confidence to work on the underlying issues which are affecting their overall health and wellbeing.  

Patient referrals are triaged internally to the most appropriate member of staff and work with patients to explore individual’s needs by developing a plan to help create positive life changes through signposting, goal setting, motivation and empowerment. We have a comprehensive service directory to signpost patients to the appropriate services, groups and organisations that would benefit them for practical and emotional support.  

The aim is to increase knowledge, skills and confidence in order to achieve their self-identified health and wellbeing goals and to help patients to have more control over their own health and find ways to improve how they feel in a way that suits them.

The Wellbeing team works for a wide range of people from 18 years + including:

  • Housing, debt & benefit issues
  • Social needs & interaction - Lonely/isolated/lacking direction in life/low confidence/low self-esteem & social anxiety
  • Low level mental health – Depression, anxiety, stress & low mood (to give direction & purpose in life)
  • 1 or more long term health conditions (e.g chronic pain, diabetes )
  • Low level addiction & recovery (getting focused)
  • Learning, voluntary & employment opportunities 
  • Lifestyle support (losing weight, exercise/move more, live well)
  • Finding a club, activity, group, or support
  • Food provision
  • Other information and advice

Service access criteria: Exclusion criteria: People with a Lack of Readiness to Engage,Currently experiencing a severe mental health crisis or immediate distress, Poses a risk of serious harm to themselves or others or living in an abusive or unstable home environment, Currently receiving care from specialist mental health services, People who have a diagnosis of, or are struggling with symptoms of: bi-polar disorder and cyclothymia ;diagnosed anxiety and depression ;psychosis or schizophrenia ;dementia ;moderate/severe learning disability ;unstable substance (drug and/or alcohol) use or dependence ;complex emotional needs ;PTSD; Patients with Personality Disorders /Self harm and difficulties with emotional regulation ,People who are housebound ,People who are already working with Carer support workers ,People on end-of-life care ,Frequent attenders to A&E / Primary care /999 It would be more appropriate to have a MDT meeting with social care and or MAT meeting The Wellbeing Team is unable to provide support for individuals experiencing a mental health crisis or in immediate distress. However, individuals with stable or managed mental health conditions may be considered for signposting to social support, recognizing the interplay between mental health and housing needs.
