SOCIAL PRESCRIBING

Social Prescribing is a non-clinical signposting service linking patients into services, groups, and organisations for practical and emotional support.

Social prescribing explores what matters to the patient, we create a plan together & empower them to take control of their situation. We spend roughly 6 sessions with them, helping to link them to various services to enable positive life changes.

Criteria: Social Prescribing works for a wide range of people from 18 years + including:

Housing, debt & benefits issues (needs housing support, struggling with forms, in debt, benefits help/advice)
Utility & financial difficulties
Bereaved & struggling to cope
Social needs & interaction - Lonely/isolated/lacking direction in life/low confidence/low self-esteem & social anxiety
Mental Health – Depression, anxiety, stress & low mood (to give direction & purpose in life)
Practical Help (link into services to help with practice problems, cleaning, mobility
Long term health conditions (chronic pain, diabetes, COPD, dementia, Sleep, mobility)
Low-level addiction & recovery (getting focused)
Learning, voluntary & employment opportunities (lost job, redundancy, finding a job, volunteering, new career, learning something new)
Mental/physical & learning disability opportunities
Struggling with parenthood & caring
Lifestyle support/ weight management (losing weight, exercise/moving more, living well)
Finding a club, activity, group, or peer support
Long Covid (signposting to self-support/management options/groups)
