Advocacy, Advice and Support for Vulnerable Adults

Families & Community Support Service (FCSS) provide a first response service for vulnerable adults/ individuals and families wanting to access social care within the provision of Care Act 2014 in LB of Newham, including advocacy, advice, information and/or support.  

1. As a first response service we carry out initiate assessment and develop close joint working with health related agencies for example Social Prescribers, GP’s and Intermediate Care, the third sector, local housing providers and other agencies, on behalf of vulnerable individuals and families in the community.

2. We work with the individuals, families and other professionals, as necessary, in order to facilitate the stated functions of the London Borough of Newham Intervention & Assessment team. This includes information gathering, the undertaking of proportionate or initial assessments, access to fast track support, signposting where required and the provision of information and advice, promoting choice and control wherever possible.

3. Identify and process any safeguarding and quality of care issues and refer on to the appropriate staff/agency to ensure that the persons’ welfare is protected.

4. Promote and empower individuals and their families and carers, to ensure they receive opportunities to access broader reablement support with a view to regaining or increasing levels of independence, as far as possible.
