Health and Wellbeing Service - For all patients (16+) registered at Brookside Group Practice or Wilderness Road Surgery only

PLEASE NOTE WE HAVE LIMITED EVENING AND SATURDAY APPOINTMENTS AND THERE MAY BE A LONGER WAIT TIME FOR THESE TIME SLOTS.  PLEASE INDICATE ALL TIMES ACROSS THE WEEK YOU WOULD BE ABLE TO SPEAK WITH THE TEAM SO WE CAN BOOK YOU AN APPOINTMENT AS SOON AS POSSIBLE.

Please add a sentence for your reason for referral so we can determine how to help you best.

We understand that not everything can be fixed by medicine and many other factors influence a person’s sense of wellbeing, so it’s key that patients find the appropriate support to help them regain control and take responsibility for their own health and wellbeing. Our Advisors work with our patients who need some coaching and support in a whole range of areas such as diet and exercise, emotional health, loneliness, financial issues and social anxiety. We are trained to help patients find the best solutions to make positive changes in their lives using manageable and realistic steps and goal setting.

We offer an initial one off 30 - 40 minute appointment (face to face, video or telephone according to your preference where possible) to discuss your situation further and make an action plan together to help you move forwards. This may be sharing resources and signposting for support taking your first steps, or may also involve some ongoing support sessions if that would be helpful.

We also work with a Specialist Mental Health Practitioner and the mental health charity, Mind in Berkshire . Depending on your needs we may offer you an assessment with our in house Health and Wellbeing Advisors, a MIND in Berkshire Wellbeing Worker or our Specialist Mental Health Practitioner.

Please complete our short referral form with details of the area you would like to focus on, and your availability and we'll be in touch quickly with a convenient appointment.  We look forward to hearing from you, and helping you to take your first steps to getting back on track.
