Social Prescribing Link Workers - Rural Alliance

We support people with social issues that affect their health and wellbeing. Examples include support with loneliness and isolation, accessing community groups, housing, employment, finances, volunteering, physical inactivity, and caring responsibilities. If the patient has a long-term condition and a social need, please go back a step and refer to the Wellbeing Coordinators (Primary Care Cheshire).
