Health and Wellbeing Coaching


Our role as Health and Wellbeing Coaches is to guide and support patients in behaviour change, so that they can gain the skills, knowledge and confidence to make positive lifestyle changes to better manage their health and wellbeing. We work closely with patients who are at risk or living with a long term condition such as high blood pressure, heart disease, asthma or diabetes, We work with patients over an agreed period of time to discuss what is important and what matters to them. Patients choose their own goals and we work together to explore the choices options and create a plan to achieve their goals.


Our service is available for patients registered at:

  • Lister House Chellaston
  • Oakwood Medical Centre
  • Lister House at Coleman Street
  • Lister House Peartree

A GP or health professional may refer to the service, or patients can self refer by providing a name and contact telephone number to one of our receptionists. You can also contact us online by using our eConsult service and one of the team will be in contact with you within 5 working days.


Health and Wellbeing Coaches


Dawn - I have 9 years’ experience supporting people to make positive lifestyle changes, including stopping smoking, healthy eating and moving more. I have also worked for a mental health charity helping people with lived experience of mental illness to set up peer to peer support groups in their community.  My hobbies include getting outdoors for a walk or a run. In my spare time I am a volunteer run leader and I love supporting people who are new to running, to achieve their running goals. I am passionate about supporting people to make positive changes to manage and improve their health and wellbeing.


Nikki - I joined Lister House Surgery this year after graduating with a MSc in Sport and Exercise Science.  In my spare time I enjoy going to the gym to keep fit, going for walks and studying a PhD in Balance Training. I have been working as a support worker, in the community and care home settings delivering falls prevention exercise classes with older adults. This is an area that I am very passionate about and I want to make a difference by supporting people to improve their overall health and quality of life.
