Social Prescribing

Social prescribers are health professionals based in GP practices. We work closely with GPs to assist patients with their non-medical needs. We do this by helping them identify, 'what matters to them'. Working collaboratively to create a care plan, which may include referrals to local services and organisations within the community.

Our hope is for patients to have a better quality of life. Ultimately supporting them to improve their overall health and wellbeing, via connecting them to community groups and statutory services for support. For example; social groups like games clubs, financial and benefit advisors, gardening clubs, sports programmes and much more.

This social prescribing service supports the following GP practices:

Service access criteria: Suitable if: • Needs support with mental health i.e. anxiety/depression and low mood and has no support in place i.e. therapy, school, early help. • Isolated / Lack of friends or family. • Not attending education and NOT engaging in support from school or has utilised school support and needs further intervention. • Social needs which are affecting their well-being. • Young Carers. • Where parent/s have mental health condition.
