Young People’s Social Prescribing

We are the Young People Social Prescribing Team of Chesterfield as well covering North Hardwick and Bolsover Primary Care Network. We work with young people age 11-18 or up to 25 if they have an additional need. We are a team of a Project Manager, Specialist Occupational Therapist and Link Workers.

Social Prescribing guides and supports young people who may be vulerable with health inequities such as: loneliness, poverty, mental health challenges, physical disabilities including obesity and much more to engage them in their communities through sport, arts, youth clubs, film, volunteering and... the list goes on!

It is not just the activities that we help young people access; we also help with transferable skills to be independent in the community after our input. We are a short term light touch service that aims to be as inclusive as possible for the young people that want to engage with us.

Service access criteria: • Age 11 – 18 or up to 25 with an EHCP • In addition, must also: • Have a home address in Chesterfield/Bolsover or; • On role at a Chesterfield/Bolsover Secondary School or; • A patient within North Hardwick & Bolsover PCN or; • A Looked After Child/Care Leaver living in Chesterfield/Bolsover • NEET living in Chesterfield/Bolsover • Attend Chesterfield Football Club Community Trust
