Children and Young People’s Social Prescribing

We work closely with Social Prescribers and School Nurses to assist Young People with their non-medical needs. We do this by helping them identify, 'what matters to them'. Working collaboratively to create a care plan, which may include referrals into local services and organisations within the community.

Our hope is for patients to have a better quality of life. Ultimately supporting them to improve their overall health and wellbeing, via connecting them to community groups and statutory services for support. For example; social groups like games club, gardening clubs, sports programmes and much more.

Service access criteria: • Age 11 – 18 or up to 25 with an EHCP • In addition, must also: • Have a home address in Chesterfield/Bolsover or; • On role at a Chesterfield/Bolsover Secondary School or; • A patient within North Hardwick & Bolsover PCN or; • A Looked After Child/Care Leaver living in Chesterfield/Bolsover • NEET living in Chesterfield/Bolsover • Attend Chesterfield Football Club Community Trust
