MSK Connect To

Connect to… is Derbyshire Voluntary Action’s enhanced social prescribing and self-care service to support people who are lonely, isolated or at risk of developing/living with, a long-term health condition.

This service helps local people increase their social and physical activity, and access emotional and practical support.

The service enables GPs and other health and care professionals to connect their patients to a range of non-clinical and community support services.

The Connect to… team includes Link Workers who are out and about meeting people in GP practices community hubs, and venues.

They are also able to visit clients in their homes and offer holistic support that may span over a few months. When appropriate they will work with clients to develop a goal setting action plan.  

Key to this service are volunteer Social Connecters who are local people with a desire to help others and to make their community a more caring and connected place to live.  A Community Coordinator and a Support and Outcomes Officer bring the service to life and support the volunteers.

Service access criteria: 18+, ability to give informed consent, knows the have been referred, needs additional support to improve their wellbeing to help manage their condition, willingness to change and engage in social prescribing
