Social Prescribing Service (18+) - SERVICE PAUSED

SOCIAL PRESCRIBING service new referrals temporarily paused.

Incoming referrals for the Social Prescribing service are currently, temporarily on hold due to having an extensive waiting list and being at full capacity.

This is a PCN decision in agreement with practice managers. The team are still working with current caseload, existing waiting list, as well as community projects, but cannot accept new referrals this time. We do not want to compromise quality of support offered to those already referred.

Comms will be circulated internally once we are able to reopen referrals, as soon as possible.
