Service access criteria: 18+ and not already being supported by CMHT or IAPT, doesn't have mental health disorder such as schizophrenia, bipolar, severe panic disorders or severe depression, isn't at risk or carrying out and completing, Patient has a diagnosis of mild to moderate depression within the last 12 months, Patient is on anti- depressant medication, Patient is currently on the IAPT waiting list, Patient also presents with mild to moderate anxiety, Patient has long-term health condition/s such as diabetes, heart disease or arthritis, Patient wants to see MHP
