Consent for Personalised Care and Support Plans (PCSP) Survey

This is a consent referral for individuals that have received a Personalised Care and Support Plan (PCSP) and are happy to be contacted by The SUN Network, to get their feedback on their PCSP. The SUN Network is an independent organisation, who encourage people with lived experiences of mental health and/or drug and alcohol challenges to have their say about local support services.  The SUN Network, needs consent from individuals to contact them in order to share a survey with them, that will hear their experience with their PCSP, which The SUN Network are undertaking on behalf of the NHS. 

The SUN Network will then contact individuals and arrange to speak over the phone, or send a link to an online survey. Feedback will be anonymous and will be included in a report that will help the NHS to look at improving the way they work. The SUN Network will be offering a £10 voucher upon completion of the survey, so please share that with individuals you are referring.

Service access criteria: Received a Personalised Care and Support Plan, consented for their details to be sent, individual has read the explanation letter and consent form, lives in Cambridgeshire or Peterborough, or is registered to a GP surgery in Cambridgeshire or Peterborough.
