Mind in Harrow Serious Mental Illness (SMI) Project

Are you aware that someone with an SMI (severe mental illness) is likely to die 15-20 years younger than someone without one on average?  The SMI Annual Physical Health Checks promotion project addresses this mental health inequality and aim to increase the number of patients attending their check.

What we offer :

Helping to overcome any barriers that patients may have to attend the health check including:

Assistance with booking their appointment with their GP
Talking through any worries or concerns they may have regarding the tests done during the session and possible outcomes
Attending the appointment with the patient and/or
Signposting them to any relevant information/ community organisations after their session if needed.

Criteria to apply:

You have received a diagnosis of schizophrenia or bipolar affective disorder, or have experienced an episode of non-organic psychosis, AND
are over 18 years old, AND
live in the borough of Harrow or have a GP in the borough of Harrow
