The Flexible Dementia Outreach Service is provided under Age UK Kent Rivers' Homecare provision and is designed to enable individuals living with dementia or memory concerns to be supported in continuing to pursue their interests, take part in meaningful activities, maintain their independence and retain or develop social contacts.
Trained support workers will spend quality time with each person on a weekly or fortnightly basis, in the comfort of their own home, helping reduce social isolation and retain or regain links with their local community.
Service access criteria:
Available for people aged 50 and over and living in the Medway area.
