Dementia Hub Outreach Team IOW

The Dementia Outreach Team gives advice and support to carers, families and professionals within the community for people with a Dementia. This is provided in nursing and residential homes, individuals’ own homes and hospital wards. We provide our experience and skills to carers and families, who are experiencing difficulties in helping people with behaviour that some people find challenging. 

We aim to provide timely, individualised person-centred interventions to meet often complex needs and behaviours that other people may find challenging. This reduces risk, improves quality of life, and relieves carer stress.  

Anyone can make a referral to the service, providing the person they are referring has a diagnosis of a Dementia, or if a Dementia is strongly suspected. It does not have to be through a GP. We have a dedicated e-mail referral system, or we can be contacted by telephone 01983 822099 EXT 5374 / 5399 / 2575
