Healthy Homes on Prescription

Healthy Homes on Prescription offers a simple, fast-tracked and non-means tested solution for small scale measures to support independent living at home.  

The Healthy Homes on Prescription (HHOP) scheme is designed to keep people from hospital, or to enable them to return home from hospital into a safe environment – This grant is used where there are no other funding schemes and  has to address hazards in the property that would present a risk to the occupier. Works would reduce likelihood of hospitalisation; examples include repairs and improvements to reduce falls or injuries in the home. A referral from a professional is required.

- Clients must be owner/occupiers 

- A vulnerable person (this could be due to a long term heath condition or disability) 

- Referral must come from a health professional who can confirm that the client is at high risk of emergency hospital admission or to enable a hospital discharge
