Social Prescribing

Social Prescribing is an holistic service embedded by the NHS, that supports people from age 16+ on the things that matter to them in their lives. When people see their GP or health professional at the surgery they often talk about the difficulties affecting their health and wellbeing and these things are often not in isolation. These could be social isolation, loneliness, mental health, housing, stress, financial and money worries, exercise, education and courses, and this list is not exhaustive so could be a combination of many of the things mentioned. It offers the chance of a confidential conversation with a social prescriber that can be used to create a simple plan co- designed with the participant that enables them to improve their health and wellbeing. We support them up to 6 separate sessions that can be face to face, either at home, your GP surgery, or community setting, over the phone or a combination of all of these ensuring they are supported to achieve their goals on the plan. Social Prescribing essentially is a social prescription that incorporates all the things that individuals want to happen in their plan, supported by a social prescriber to address these, which improves their health and wellbeing.

Service access criteria:

Aged 16+ and must be registered at these following GP surgeries, Danes Dyke, Hackness Road, South Cliff, Filey and Hunmanby. Must not be actively engaged in Drugs and alcohol or present as in crisis or suicidal or at risk of self harm.
