Adult Social Care Social Prescribing Team, Enable

Working in partnership with Wandsworth Council Adult Social Care's CAST and First Contact teams.

Using motivational interveiwing and health coaching tecniques we identify 'what matters to them' and work collaboratively to create goals which may include referrals to local services and organisations within the community.

Our hope is for clients to have a better quality of life and retain their independence for as long as possible, ultimately supporting them to improve their overall health and wellbeing by connecting them to community groups and statutory services for support.

All appointments are conducted over the phone; in a small amount of cases we can do home visits - criteria for this is strictly to aid communication.

In cases of difficulties with housing, please manage expectations of clients - we will not be able to be able to facilitate an immediate change in cirumstances.

 

Exclusion criteria:

  • Those with severe and/or unstable mental health conditions
  • under the age of 18
