Self-Management Course - Expert Patient Programme

The Expert Patients Programme (EPP) is a free course for people living with long-term health conditions. It aims to help you to improve your health and wellbeing and better manage your long-term condition through learning skills and techniques that you can use in your day-to-day life.

The course is run in a group setting over 7 weekly 2.5 hour sessions. The first session being an introduction to Microsoft Teams and general tech advise for the participants. It is led by tutors who manage their own long-term health conditions, with the help of the techniques taught on the course.

Some of the topics covered on the course include:

dealing with pain and fatigue
dealing with difficult emotions
planning and setting goals
healthy eating and exercise
communicating with family, friends and health care professionals
finding other health services in the community
To join a course, you must be aged 18+ and live, work or be registered at a GP practice in the boroughs of Merton or Wandsworth.
